Team members from KidsInclusive - Centre for Child & Youth Development’s School Age Team will highlight how they work as a multidisciplinary team to help support children and youth through several transitional periods.  Through case examples and interviews they will discuss the real life issues that accompany the Transition to School to Entry and the Transition to Adult Services.  They will discuss roles of each team member, how the team collaborates with community agencies and school boards, and the protocols and tools that have been developed at KidsInclusive to help support these transitions.

Utilize principles of effective team building and shared leadership to cultivate and sustain healthy team relationships
