What is your personal experience with equity, diversity and inclusion? What is broken? What do you want to see changed? And importantly, how should we fix it? Please use the form below to share your story.
Completing the form is anonymous, however you are also welcome to provide your name and information. To protect your privacy, submissions will not be posted publicly. This form will remain open as we undertake this work, and the DAT-EDI will review every submission.
